Acknowledgement and Dedication

Once upon a time there was a lady named ANGELICA GONZALES MASCOLINO, 18 years of age a DOMT student at Polytechnic University at the Philippines who needs to accomplish a 250 hours of internship at the hospital under as her requirement in her OJT 2. Now that she has completed her internship she would like to acknowledge Mr. Arjen E. Bagongon as his supervisor  at Medical Records Department. Sr! Thank you because you have part of this journey thank you for all the things and for all the knowledge you shared with me, I believe that it will be helpful whenever I have my job in the future. And to my loving OJT Coordinator Prof. Sherylen Morales, thank you for providing us the knowledge regarding medical that really helps us in our internship. And to the Lord, God Almighty, thank you for providing us what we need to accomplish this. Indeed, all things are possible with you.

I would really like to dedicate this to my family who is really my inspiration. I believe that it is really a step of accomplishment as a student until I come where I will wear my toga and I will get my diploma. There is a saying "There is no elevator to success you have to take the stairs" and OJT is part of the stairs I must take up for me to success!

Again, thank you for those people who have been part of my beautiful journey. May the Lord bless us abundantly! :)

One day, I used to start my internship at Brigino General Hospital. At first, it makes me feel and think negatively during my internship but as the day goes by I have learned so many things and this certficate proved that I have completed the 250 hours of requirement. When I see this certificate, I really feel good because it proves my determination to accomplish things i have started.



MY LAST WEEK OF INTERNSHIP AT BRIGINO GENERAL HOSPITAL

As an intern at Brigino General Hospital, I must say that I have learned so much. My last week really made me so determined in accomplishing the tasks they ask me to do. but this time, it is more challenging for me because I do not have a partner in doing things and my supervisor is not in the area to supervise me for he have to attend a seminar in Pampanga for one week. The good thing is I used to measure my skills in terms of time management because there are some things that has to be done with the given deadline, so I really work effectively under pressure. At the end I evaluated myself and I could say that this experience to work in toxic really made me feel as if a real staff in the hospital and not just an intern.

MY DAILY TIME RECORD


Photos below are my daily time record (DTR). There you will see the time of my in and my out. I must be in at 8AM but unfortunately, sometimes I am late. As you can see, my some of my record doesn't have the sign of my supervisor it's because he is not in the office to sign for he was assigned by the administrator to attend a seminar held at Pampanga and he will come back, Saturday (June 11). And as I total my number of hours, over all number of hours is 255.25 hours that suits my required number of hours of 250.

This is how we arranged the out patient medical history by its case number.

This is how we arranged the patient's medical chart. Of course, alphabetically and it must be on its type, whether it is for a medical, pedia, or ob. Very simple right? :) As you can see, there is a star in the title of it, meaning the patient has its medical membership most probably a PhilHealth member.

All About Medical Records Department

Since I was assigned in a Medical Records Department supervised by Mr. Arjen Bagongon, let me give you a little background about it. The terms medical records, health record, and medical chart are used somewhat interchangeably to describe the systematic documentation of a single patient's medical history and care across time within one particular health care jurisdiction. The medical records includes a variety of types of "notes" entered over time by time health care professionals, recording observations and administrations of drugs and therapies, orders for the administration of drugs and therapies, test results, x-rays, reports, etc. The maintenance of complete and accurate medical records is a requirement of health care providers and is generally enforced as a liscensing or certification prerequisite.

OJT Tasks Evaluation Sheet

In our internship, we are not just going to be "pa cool-cool lang" because we are having an internship for us to learned and adopt new things. And for us to be able to do so, our supervisor needs to observed us in whatever we do. So that whenever there is a room for improvement or we have some errors he can immediately tell it to us. But in the system of the school there is such thing called "OJT Task Evaluation Sheet" where he needs to evaluate us by rating the said duties from 5 which is the highest and 1 which is the lowest. But there is also a column for not applicable. Here is the sample of the task evaluation sheet. See below.

PS. My supervisor have already evaluated me and I've already passed it to my coordinator! Thanks be to God for the good evaluation/grade. :)



Recommendation Letter and Waiver




Photo's above are one two of the primary requirements when you are applying to be an intern in a company or for me (in a hospital). As you see, the recommendation letter needs to be addressed by the administrator or any person who will be supervised your internship. In my case, I've addressed it to the hospital's administrator Mr. Leo v. Brigino but he is not the one who signed it for the received signature. My supervisor, Mr. Arjen Bagongon signed my recommendation letter even my waiver because he will be my supervisor in my internship for the said hospital. In the recommendation letter, it tells the duties and responsibilities of the trainee but not limited to the stated above (in the recommendation letter). It also tells the schedule and minimum requirement of 250 hours. Of course! It was signed by our Training Coordinator PROF. SHERYL R. MORALES and it was noted by our Academic Head, PROF. DORIS B. GATAN. While in the waiver, it must be signed by the trainee which is me and my parent and was noted again by my supervisor.

Brigino General Hospita's Logo

This is the logo of the hospital. At the first thought, you will just say to yourself that this logo is so simple but has it's deep meaning. Snakes then symbolized rejuvenation. Many mistakenly think that the Caduceus (a short staff with two snakes and a winged up) is the symbol of medicine, or call the Aesculapius staff caduceus. The caduceus is the symbol of Hermes (Roman= Mercury) the messenger and symbolizes travel, commerce, etc. While this may not be all of it, some of the confusion traces back to around 1920 when the US Army mistakenly used the Caduceus for their medical personnel. Very interesting right? So that this time, I think that this hospital used this logo because of the interesting meaning behind it. But now, as I also think about... What does the snake and the sticks have to do with the doctors? Until I've come to the answer, If you ever seen the World Health Organization or the American Medical Association logo, or the "star of life" on the side of an ambulance, you might have wondered what a snake wrapped around a stick has to do with those who fix ails us. Well, that stick is the asklepian, or rod of Aclepius. In ancient greek mythology, Aclipius was the son of Apollo, and god of medicine and healing. Depending on which historian you ask, he may have been based on an actual historical doctor whose skills became so exaggerated that patients formed cult around him.

With my blog you can see what happens during my almost one and a half month internship and brigino General Hospital. Oh! Let me give you a little bit of information about this hospital, Brigino General Hospital is located at 20 Quirino Highway, Highway Pecsonville., City of San Jose Del Monte, Bulacan. It is a liscence DOH PhilHealth accredited hospitals. Hospital's vision and goal is to provide high quality of medical service and safe delivery of mother and child by a competent, compassionate and ethical health professionals while it 's mission is to render quality health care through effective teamwork. Hospital's administrator is Mr. Leo brigino, RHP, RN. I am assigned to the Medical Records Department and with the supervision of Mr. Arjen Bagongon, MrDept / IT Head.
